What Is Transitional Care?

Transitional care is specialized non-medical assistance provided after a hospital discharge or major health event. Our caregivers ensure a smooth and safe transition by offering hands-on support, emotional reassurance, and coordination with healthcare providers to reduce the risk of complications or readmission.

Services may include:

  • Assistance with discharge instructions and medication reminders

  • Personal care (bathing, dressing, mobility support)

Dementia Home Care

Senior Fall Risk Management

Fall prevention in the home refers to measures taken to reduce the risk of falls among seniors and other individuals with limited mobility. These measures can include things like installing grab bars in the bathroom, removing tripping hazards from the home, and providing adequate lighting throughout the living space.

Pre & Post Surgical Support

We will help your loved one prepare for surgery, and help close communication loops with the family, agencies, and doctors. We will also visit our client at the hospital and provide companionship. Once home we monitor compliance with doctors’ and other healthcare practitioners’ instructions and medications.
